The aim of this study was to determine pharmacokinetics (PK), tolerability and effects in the metalloprotein matrix-9 (MMP-9) and the soluble-1 intercellular adhesion molecule (sICAMP-1) of oral LA in patients with EM. Thirty-seven MS subjects were randomly assigned to one of four groups: placebo, LA 600 mg twice a day, LA 1200 mg once a day and LA 1200 mg twice a day. The subjects took study capsules for 14 days. We found that subjects taking 1200 mg of AL had levels of L. of peak serum substantially higher than those taking 600 mg and that the maximum levels varied considerably among subjects. We also find a significant negative correlation between the maximum levels of serum L. and the average changes in the levels of serum MMP-9 (T = -0.263, P =0.04). There was a significant dose response ratio between LA and the average change in serum sICAM-1 levels (P =0.03). We conclude that oral LA are generally well tolerated and seem able to reduce the levels of serum MMP-9 and sICAM-1. LA may be useful in treating MS by inhibiting MMP-9 activity and interfering with T-cell migration at CNS.
